You should always include a covering letter with CVs or application forms, this letter draws together and recaps everything that has been included in your CV or application form and gives the recruiter an opportunity to see clearly why you are the most suitable for the job.
Here are the basics

One page of good quality A4 paper

Letter shouldn’t be hand written

Letter should be addressed to a named person rather than Head of Personnel

Should include what job the application is for and where you saw the advert

Main section should provide an overview of why you are interested, what you can offer highlighting your suitability for the role

Use the appropriate close - Dear Name…Yours sincerely and if you have to Dear Sir/Madam…Yours faithfully
